It's easy to not bother calling the EA hotline to report an incident.
You call to report something. Nobody comes out, nothing happens. Next time you don't bother.

Today I was banged to rights on this by a presentation from one of the EA water quality officers.

Everything is logged. Even if no-one comes out - the information is kept on the database forever.
If you see a sewer discharging but it looks like a one-off - call it in.
When you see it happening again the following week - call it in again.

The EA have limited resources and, even if they appear to be doing nothing about your incident, for example in the case of mis-connections they will have reported it to Thames Water -who have a specific budget and procedure for tracing and fixing these.

Sometimes discharges only happen at a particular time of day, if you regularly see something "wrong" then call it in, straight away, every time.

The most important things are to get the time, and an accurate location.
If you can get a photo on your mobile, even better.

Multiple calls help to build up a picture. Always call it in!

I work for a big organisation with similar issues. Put the complaints in even if you have no response! It is how big orgs identify problem areas...

I think it was/is the case that if they receive the same report on 3 separate occasions then they are obliged to investigate. Can anyone confirm or refute this?

I can't confirm whether it was the case in the past and it wasn't mentioned explicitly Neil but it was made clear for sure that if they get multiple calls for the same location it raises the profile so it will get noticed and action will be taken eventually.  
Often an outlet may discharge only at certain times, so if they randomly went down there nothing would be found.  She gave an example of a dog walker who called a number of times because every Monday at 9am an outlet was discharging something, it turned out to be some builders pumping out a basement which kept flooding over the weekend into a rainwater gully.
